Forms and Formats
The Pennsylvania Department of Education has drafted forms related to all aspects of special education to assist parents in understanding the Department's procedures and processes. The following publications will help in the development and documentation of legally compliant evaluation reports, individualized education programs, and notices to parents. The annotated versions of these documents contain "best practice" suggestions for preparing the documents that highlight certain sections of particular importance.
Evaluation ReportThe following sample Evaluation Report (ER) includes annotations that are useful, once the multi-disciplinary evaluation has been completed. Parents are entitled to a copy of the Evaluation Report prior to the IEP meeting. The IEP team considers the recommendations contained in the ER in the development of the IEP.
Individualized Education Program The annotated Individualized Education Program (IEP) is designed to assist IEP teams with content development, and ensure compliance with Individuals with Disabilities Education Act (IDEA '97 ) requirements. School districts should have begun to use the revised IEP format as soon as possible, but were required to use the new format for any IEP developed on or after September 1, 2004. PDE made additional revisions to the annotated IEP forms by highlighting certain sections of particular importance.
Please Note: The yellow highlighted versions (see below) include the revisions BSE has made to the annotated IEP. All local education agencies should review the highlighted areas carefully and ensure that they incorporate this required information in all Individual Education Programs.
Notice of Recommended Educational PlacementThe Notice of Recommended Educational Placement (NOREP) serves as "notice" to parents and meets the requirements of 34 CFR 300.503, and is also used when seeking informed consent.
Early Intervention FormsThe following forms are for use in writing Individualized Education Programs for children enrolled in pre-school Early Intervention programs.
